- - 📌 I KNOW HOW TO MAKE IT BETTER. He had an accident during an MMA fight seven months ago. The left tiba & fibla bone was fractured after a kick attack on the opponent. He underwent surgery and is currently undergoing rehabilitation. It was a big injury to him. Long hours of post-op rehabilitation have reduced his performance, poeer and muscle strength. DAY 2 POST-OP: A little pain when standing etc and doing heel slides, but nothing crazy. Haven’t took any analgesia yet today. I’m complete Non Weight Bearing for 2 weeks. Today’s exercises have included: 1. Static quads 2. Heel slides 3. Electrostim machine 4. Ankle pumps My earlier post on the usefulness of additional exercise via electrostim devices post-op has been 100% proven to me by the painless and strong contraction I’m able to get through VMO and the quads in general in these early stages. As anticipated I lack full extension (also encouraged to avoid for graft protection) and quad activation. *Regarding heel slides - any activation of hamstrings and thus the graft site is PAINFUL so doing a little more passively at present. I’m using a really simple slider disc on the video that you may see in the gym but you can use socks and a slippy surface just the same. . . Today we continue discussing a very controversial topic of ACL rehabilitation: ACL re-injury❗️❗️ . 👎👎 After years of ACL surgeries the outcomes following these long-term injuries are still unsatisfactory compared to other injuries. . 🛑 Approximately 50% of patients do not return to the same competitive level of activity after anterior cruciate ligament reconstruction (ACLR), and as many as one third of young, active patients with ACLR sustain a second anterior cruciate ligament (ACL) injury within 2 years of returning to activity. . 🆘 ACL re-tear rates are approximately 20-25%. That means one in every four to five athletes are suffering a second ACL tear. . 🤷‍♂️ There is no clear consensus on when the player is ready to play after an ACL injury. ✅ Training load: a key factor in ACL rehabilitation. . 👉 Load progression is a key part of every rehabilitation protocol. . ❓Too much too soon rehab / training may increase risk of reinjury , not enough rehab / training may also increase risk. . ➡️ Basically what we are trying to do here is to expose these patients to enough practice and repetition of the skills necessary to play to allow the body to re-adapt to the sport. . Unfortunately this process takes time and knowledge. . We follow this general progression of : linear movement, multidirectional movement, soccer-specific technical skills, soccer-specific movements, and practice simulation, practice with the team, scrimmages, 20-30 min of real game time, half- game, full game. . This continuum of progressively challenging the repaired ligament and the kinetic chain will expose the player to sufficient repetitions to match the demands of the game. . 👉✅ Prolonging rehabilitation and RTS might reduce the risk of subsequent injury by nearly 5-fold because it allows more time to re-train the player. . Knee UPDATE: 5 weeks post off and it’s official IM OFF CRUTCHES!! Saw my consultant yesterday and he is super happy with my progress. All the hard work i spent with my coaches and physio for pre op prep is now paying off! He signed me off my crutches and has given me the go ahead to now start working on my flexion, get on the bike, cross trainer and start building up some weights. The only no go being breaststroke and a weighted full lunge past 90 degrees. The graft is apparently solid and my scars healing well. Meanwhile physio just got a whole lot harder! Never have I ever felt pain like stretching of a quad post op! But no pain no gain! Now time to get on the bike and get that knee moving! My one piece of advice for anyone going through this or something similar is spend the time doing the pre op prep work! It might be hard or seem pointless but it’s worth it’s weight in gold the other side! Versucht einen kompletten Kreis um euch selbst zu ziehen und dabei stabil stehen zu bleiben. Dabei müsst ihr teilweise ein kleines bisschen in Knie gehen. 2. Ganz langsam und konzentriert in eine Standwage gehen. Dabei sollte euer Oberkörper immer Richtung Boden zeigen und sich nicht verdrehen. Danach ohne Absetzen das Knie vorne nach oben bringen. 3. Einen Ausfallschritt nach hinten machen und das Knie dann ohne abstellen nach oben zur Brust bringen. Ich mache immer 10 Stück pro Seite und wechsel dann. 360 tabes during the KDC BMX demos ~ @afreakineric Wouldn’t think after having such a rad sesh out in Kimberley I would need more surgery😅 but MRI’s don’t lie. Trap bar deadlifts contribute to lower peak moments at the lumbar spine, hip, ankle, but an increased peak moment at the knee. It is more quad dominant than the conventional deadlift and a more upright position may be less taxing on the lumbar spine. - The trap bar allows for training around injury but with relatively high loads. The disadvantage of a traditional barbell deadlift is that the forward position of the barbell causes a less than optimal moment arm to stabilize the center of mass to be in front of your body and so your axis of rotation is a greater distance away from the load. That causes increased shear forces in the lumbar spine, which puts you at greater risk to injure the discs and surrounding structures should your form be less than optimal. - So why the trap bar? - 1️⃣Great for beginners 2️⃣Decreased learning curve 3️⃣Decreased Shear Forces on Lumbar Spine 4️⃣Able to Lift Heavier Loads or more quickly for power development 5️⃣Shoulders and hands remain in a relatively neutral position as opposed to mixed grip setup 6️⃣More forgiving on the postural mechanics of a combat athlete (think upper crossed syndrome) in that we tend to keep a thoracic flexion curvature as a functional position during a fight 7️⃣More of a hybrid squat/hip hinge so may feel like a more “natural” movement to a grappler 8️⃣Versatility - can function as a deadlift, grip trainer, dynamic stabilizer/core blaster ie farmers walk

Risk of knee osteoarthritis after different types of knee injuries in young adults: a population-based cohort study 🦵🏻 . ▫️This great study looked at 5200 people with knee injury and 140,000 people without knee injury. . 🕵🏻‍♂️ Authors examined: . ▫️The relationship between knee injury & knee OA. . ▫️The interaction of age and gender. . ▫️And the risk of knee OA after knee specific injuries. . 📚 The big take is that ACL and meniscus injury appear to increase the risk for knee OA versus those without knee injuries, with women affected a bit more than men. A bigger question is why is it that these injuries lead to OA? Altered joint mechanics? Kinesiophobia? Reduction is physical activity habits? All the above?

Infelizmente, Yang Fang Xu rompeu a ACL nesta aterrissagem. Ela tinha deficiências na mobilidade do tórax e ombro, o que significava que quando ela chegava a bater, ela tinha que flexionar o lado para a direita. That meant landing on two feet wasn’t possible. With very limited right ankle dorsiflexion, a medical meniscus injury, hip and low back pain, the control of landing forces was not possible and the lower limb valgus event occurred. When we know to look for that which is below competent, “a potential signature of failure”, we have the opportunity to change it, and stay in the game where the athlete can continue to apply their strengths - their “potential signatures of success”. #ACL #movement #sportsphysio #sportsphysicaltherapy #sportsphysiotherapy

ACL Hamstrings —- Regaining hamstring strength is a huge part of successful ACL rehab. The hamstring is a dynamic knee stabilizer, which makes it a pivotal component of rehab. Many people ask what graft type is better and using a knee stabilizer as a graft is one of the main arguments against it. I always want to ensure my patients are adequately challenged in all phases of rehab. —— Here are two hamstring variations I like to use at different phases of ACL rehab. When can you do these? When you’re ready! 1️⃣ Single leg hamstring curl with yoga wheel - single leg hamstring exercises are must in the later phases. The yoga wheel adds a challenge compared to a ball as she has to ensure it doesn’t tip over. 2️⃣ Banded sorinex glute ham rolled hamstring curls - one of my favorite pieces of equipment for hamstring strength of as it can be used as an assessment as well. When one hamstring is stronger, the roller will start to tilt to the stronger side. —— 🧠𝗟𝗼𝗼𝗸𝗶𝗻𝗴 𝘁𝗼 𝗹𝗲𝗮𝗿𝗻 𝗺𝗼𝗿𝗲 𝗮𝗯𝗼𝘂𝘁 𝘀𝗽𝗼𝗿𝘁𝘀 𝗿𝗲𝗵𝗮𝗯? 🚨ACL Rehab🚨 🏀Return to Sport🏀 🚨Reactive cutting🚨 🏀 In the late stage of rehab we begin to make the task more complex by adding reactionary component. The athlete must be prepared for the open environment when they have to react to external stimuli in a closed environment meaning with a coach before going to an open environment. 🏀 Biggest key points is that we are trying to help the athlete feel safe in his return to sport activities.
