What is Chondomalacia Patella (CMP)? . CMP is a condition in which the undersurface of your patella (or knee cap) has it's cartilage deteriorated and softened. It often occurs in athletes as an overuse injury, but may also occur in older individuals. This condition may be related to osteoarthritis in the patellofemoral joint in the older demographic too. Other than pain, a grinding sensation may occur with audible crepitus (grading, crackling, or popping sounds). . So how did this happen? The usual culprit is due to improper joint mechanics or due to poor load/intensity management. Knee mechanics such as knee valgus or lateral patella tracking (when overly stressed) can cause grinding of the under surface of the patella against the femur which leads to the degeneration of the cartilage. So in order to address the exact issue for you (beyond the pain) it is important to get a thorough assessment to look at your movement quality. . Most of the exercises we go over in this post will address the lateral patella tracking issue. We try to encourage medial or inner movement of the patella by doing exercises that recruit your Vastus Medialis Oblique (VMO) muscle which is responsible for pulling the knee cap inwards. My little workout dude asked to join me yesterday. 🥰 _ For as long as he can remember, he knows that mom workouts. He was 16 months old when I got my first program. _ Before that, I was playing soccer and going running... which I did NOT enjoy the running. I just did it because it was what I always knew. _ I had to change it up when trying to run with 4 little was causing more stress than reducing it. Or I had to wait til Jimmy got home so I could go by myself...I was at the mercy of his schedule. And, by that time, I might have been about to lose my mind. Or maybe I already had. There were times where Jimmy would come home, take 1 look at me, and know I had enough that day and to go running. 🤪 _ I LOVE that my boys see me workout. I LOVE that this has lead to conversations about taking care of yourself to be healthy and strong. I LOVE that they seem me struggle with hard workouts and watch me keep trying. I LOVE when they encourage me to keep going. I LOVE that they can join me. I LOVE that I can do it on my time. _ This is normal to my boys. They know I need my time to workout to be a more calm and patient mom. They know it’s important to me and I LOVE that I’m showing them that as they get older, they need to take care of themselves too!!! _ Mamas, I know you are tired. Maybe even exhausted. But pain? Knee pain sucks 😣! But don’t let that stop you from training legs 🦵. My client (shown) has a history of knee pain, and this has been a staple. I call it an Offset Split Squat with TKE (terminal knee extension). I know, a mouthful 😯, but never mind the name. What’s important is that you stop 🛑 skipping leg day! Begin by placing a band around a rack or something stable. Place the other end of the band around the back of your knee. Split your stance wide enough to form a perpendicular/vertical shin angle with the leading leg. Lower under control, pause for a second at the bottom, and return to the top. At the top, focus on extending your knee and squeeze your quad hard. Start off with 2 sets of 6-10 reps. Twelve months post-op. 🙌🏼 . It's been a full rotation around the sun since I was in surgery for a torn ACL. I've made so much rehab progress within the year; going from feeling stuck in bed and having to use crutches to get around to learning to walk again by slowly placing one foot in front of the other. . I remember the days of constant heel slides, quad sets, and leg lifts; the discomfort of trying to cycle; walking up and down stairs one step at a time. I remember making progress and feeling so accomplished for small feats yet realizing I still have a mountain ahead of me climb. . A year may have passed but the work doesn't stop now. I still have a lot more jogging, plyometrics, and squats ahead of me. . Glad to be one year into my progress and feeling strong. . Special thanks to @pctru_fitness, @Zensual, and @Dallasthenics during this recovery journey. I feel like this is some kind of an acceptance speech. Lol. . Psychological rehab must not be overlooked when rehabbing a physical injury. Our bodies protect and guard. They are untrusting of our capabilities and and are hypersensitive to any sensation. This leads to lack of confidence, compensation and poor mechanics. None of us should settle for “it’s doing okay”, but a #professionalathlete definitely does not have that luxury. Reactive timing, proprioception, speed, agility, endurance, power and range of motion are crucial to playing a sport. After injuries in close succession, the self doubts and fear are even worse. So what to do? Let the body prove to the brain over and over again that it’s strong, capable, pain free and ready! After proving to your brain consistently and verbally rewarding your accomplishments (no matter how small), chemicals in the brain will not only like doing it, it will want to perform the task more- and I let it! I want my athletes (all my clients) to walk out elated, accomplished, stronger, in less and preferably NO pain!! Chronic Knee Pain Relief: This 30-year-old female patient has been suffering from knee pain for five years. She has had negative MRIs and x-rays, and no relief with physical therapy and other treatments including rest. She is a very active individual who eats right, and also exercises with a personal trainer who is aware of her condition. I was so happy to hear that all of her radiology images were negative. Since we've already confirmed the menisci and ligaments are healthy, we could then evaluate her biomechanics of the hip knee and ankle to see if multiple regions of her body were contributing to the chronic knee pain. As shown in the video, we deciphered that the main cause of her pain was repetitive use of her left quad in day-to-day activity getting in and out of her truck followed by intense exercise without propee rehab techniques to the left knee soft tissues. This is why she can't pinpoint where her pain is, because she's developed repetitive inflammation and chronic adhesions in the distal quads, gracilis and sartorius (inner leg muscles), and IT Band. We utilized instrument assisted myofascial therapy seated, then in weight bearing squated positioned to reproduce pain and generate immediate results. She was able to walk and do squats with a significant reduction in pain after 5 min of treatment. 5-6 treatments over the next 4 weeks was recommended, as well as at-home pre and post workout routines, and supplemental help such as turmeric to reduce inflammation.
