HAMSTRING STRAIN ON A COLLEGIATE SOCCER PLAYER This soccer player presented with persistent tightness and pain in his high medial right hamstring. He injured the area while playing when he lunged for the ball, stepped on it, and it went out from under him in a sudden and forceful manner. He also pointed to an area in the right buttock, around the sacrotuberous ligament and lower deep hip rotators, as a region where he continued to feel pain. Here are some of the exercises we gave him to start doing daily at home. Look out for a post showing some of the manual therapy we used as part of the treatment plan. Deep Squat while Holding On: great full body movement for ankle, knee, hip, and spine mobility. The more you can keep the spine upright, the feet parallel and facing fwd, and decrease your dependence on holding on to a solid object the better. Split Squat: classic go-to exercise. Works the hamstrings and glutes. Squat for Ankle Mobility: what became evident as we were working with him is that he had a very tight Achilles-calf complex on both sides with significantly limited ankle mobility. This is common for a soccer player. Because of this you can see in this exercise how limited the depth of his squat is as well as how difficult it is for him to keep his legs from turning out. Palms on Floor Hamstring and Calf Stretch: if you have good flexibility in the posterior chain of your legs then this exercise is relatively easy and pain-free. 1) Glute Ham Raise with Isometric Holds . Recent studies(*) argue eccentric moment on hamstrings during swing phase of running is not as significant as previously believed. These studies have found greater isometric hamstrings activation. They suggest isometric training is more effective at conditioning the hamstrings for high speed running. . While I'll continue training my hamstrings eccentrically as it provides various other training benefits, I'll also be sure to incorpotate more isometric holds into my programs. I actually completed these after Single Leg RDLs and my hamstrings were screaming!! . 2) Box Front Squat . Controlling range of motion is a good way to continue training around aches and pains. Box squats can serve many purposes beyond ROM cues. They also provide greater posterior hip movement, placing greater demand on glutes and hamstrings. When used with pause (not done here) it decreases contribution of stretch shortening cycle for better concentric workout. . I like to include these early on to prep for Olympic lifts such as Cleans. Squats are great but make sure to incorpotate variations that help build to your end goals. For example, when working with novice squatters I recommend unloaded overhead squats to teach proper mechanics and progress to sport specific variations. . *Hooren, Bosch 2018. When performing a medical screening for BFR exercise, it is important to identify if these are present, as it will influence whether or not BFR should be included within the individual's plan of care. 🔸 In the above example, the participant has varicosities (as identified by superficial, miscolored vessels) but her diagnosis (Inclusion Body Myositis) and the progression of her diagnosis (muscle wasting) and an inability to lift traditional recommended 70+% 1RM exclude other options. Downward Dog To Baby Squat Down Dog To Baby Squat uses the strength of your arms and legs to improve mobility of your shoulders, spine, hips, knees and ankles. It stretches your shoulders, back, hips, hamstrings, and calves as it strengthens your shoulders, core, glutes, quadriceps and ankle stabilization musculature. It opens your chest and shoulders and tones your arms and abdominals. 10 Tips To Downward Dog 1. Hands should be shoulder width apart. 2. Feet are a little wider than hip width apart. 3. Activate your arms. 4. Upper arms externally rotate. 5. Neck and head continue along the same line as the spine. 6. Firm shoulder blades and broaden across the upper back. 7. Engage the lower belly by drawing the navel in towards the spine. 8. Bend knees a little (or a lot) and send the sit-bones and tailbone up and back. 9. Inner thighs rotate inwards as you firm the outer thighs. 10. Straighten legs without changing the shape in the spine or pelvis with feet slightly wider than hips width apart. 3 Tips To Baby Squat 1. With your feet slightly wider than hips width apart. Angle your toes so that they point forward or slightly outwards and MOVE from Down Dog into Baby Squat. Note: There are a few things that contribute to your squat stance. If you have tight glutes, toes pointed slightly outwards might be more comfortable. This stance also allows you to take on more weight (i.e. weight lifting). 2. Engage the muscles in your core, legs and glutes as you lower your butt down and back (like you are sitting in a chair). Keep your weight in your heels, press your knees wide as you go down and ensure your knees don’t track ahead of your toes. 3. Sit to your lowest point, without letting your pelvis round (what I like to call a “butt wink”). Maintain a straight spine and lifted chest. Note: Hip and ankle mobility might impact how low you can go. 4. Return to standing by driving down through your heels and squeezing your glutes as you come up or transition back to Down Dog from the Baby Squat. Everyone’s anatomy is different. Your depth or stance might be different than the next person’s. Follow these tips to prevent injuries and have fun!

Being sick is hard. It’s tiring, frustrating and downright cruel at times. I’ve spent more time in the rehab centre of my hospital in the last year and a half than I ever thought I would in a lifetime before getting sick. One area I really struggle with is physiotherapy. I’m gonna go on record here and just say it out right: I fucking hate physiotherapy 😂 Don’t get me wrong, I’m beyond grateful for the amazing physiotherapists I’ve worked with in the past and present (I’ve been referred back to the physio dept. I start January!). They have all of my respect for the amazing work they do but it’s still so hard. Here’s why I dread every appointment: 1. I’m so fatigued at the end of the day that I want to just collapse into bed and fall asleep and even the thought of doing physio makes me cry. 2. It hurts. I feel nauseous, my body’s trembling and my muscles are aching and my joints are giving out and dislocating and it just hurts. 3. It’s frustrating. I feel like I can never get any results, that if I tried harder then I’d be doing better. I feel like I’m letting my physiotherapist down by not progressing as quickly as I “should” be. It’s that chronic illness guilt we’re all too familiar with. Yet despite all of those, I carry on. Why? * Because this could very well be the only shot of improving my prognosis by stabilising my joints. * Because I want to avoid surgery. * Because quitting has never been in my bones (neither has collagen 🤣) and I’m not about to start quitting now. I carry on because I’ve lost so much to EDS already. I really don’t have that much more to lose but I sure as hell have a lot to gain back. 📷 Strasbourg, France. Trigger warning. I've been backing up my phone recently and found these old photos of Bean that had be 'lost'.. Well forgotten. Cos I don't really want to be reminded of how poorly he got. I've just burst into tears. Seeing his eyes sunsetting.. Being told that it wasn't sunsetting and that he'd have to wait for his surgery.. Even though it could have caused more damage. I fucking hate these pictures. But as much as I should delete them again. The serve as a reminder of how far my baby boy has come. He has changed so much. He doesn't even look like the same baby from the day he was born.. To these photos.. To now. The change is unreal and I love this little sausage more and more each day even if he doesn't like to sleep. I am not ready to see him so vulnerable again but I know it's going to happen. I know he's at high risk of getting an infection when he gets his peg fitted next year. I can't watch him go through more and more surgery. This isn't the life I wanted for my baby boy. Quadriceps tendinopathy with partial tear. Patient came in with very focal tenderness over the insertion of the quadriceps tendon. Symptoms are triggered mainly by#running and #squats. Under US you can some classic features of tendinopathy. Notice the appearance of the tendon is not the same throughout. Particularly around the sprinkles area the tendon is bright and then dark. This signifies the quality of tendon is not uniform througjout. The finger is also pointing to a small partial thickness tendon tear. The side plank! 💪🏽⁣ ⁣ An exercise that often comes to mind in order to strengthen the core. ⁣ ⁣ However, did you know that this exercise is also great for strengthening the gluteus medius? (The same muscle that we use to keep our hips level - see last post! 😉)⁣ ⁣ This exercise can be modified to suit many skill levels:⁣ ▫️Beginner: Bottom knee on the ground⁣ ▫️Intermediate: As shown above⁣ ▫️Advanced: Challenge yourself by adding a hip dip, a leg lift, or an arm open/close⁣ ⁣ This is a great exercise for runners 🏃‍♀️ as well!⁣ ⁣ Give it a shot and let me know how it goes!

Regressing standing one leg transitions part 2 (part 1 was yesterday if you missed) . A key principle that physical therapists use in rehabilitation is to break down and build up movement to create strength stability and control in areas that need it . Giving the body experiences of doing the same movements and musclular work in regressed versions of the ultimate goal movement is a useful way to build up resilience, capacity and preparedness . We can use this in yoga where we break down asanas into more regressed versions and build them back up. (That’s a keystone of our training) .
